Starting Monday, April 12th, Illinois will be moving into the next phase of COVID-19 vaccine eligibility, Governor Pritzker announced. Any Illinois resident who is over the age of 16 will now be eligible for the COVID-19 vaccine.

In order to receive the vaccine, you must make an appointment.

For Rockford: A single dose vaccination site will be set up at Allen Chapel AME Church (3000 Rural Street) from 9 am until 5 pm. Click HERE to make an appointment.

For Ogle County: A mass vaccination clinic will be set up at the Oregon Fire Department. They will be administrating the Johnson & Johnson vaccine. Click HERE to make an appointment.

For Boone County: A site will be set up at the Poplar Grove Fire Station (305 W. Grove Street) starting from 830 am until 330 pm. Click HERE to make an appointment.
